• The moment you enter the office go to the registration desk
• Hand over your ID card and Child Bay form to the representative at the registration desk.

• The computer operator in the registration room of the survey will ask you several questions and assist you in filling out the form.
• After you fill out the registration form, your fingerprint will be taken for verification
• Your survey registration process will be completed and you will receive a confirmation SMS also
