- The applicant must be a permanent resident of Punjab.
- The applicant must possess a special disability certificate from the District Disability Assessment Board.
- The applicant must possess a special identity card for disabled individuals from NADRA.
- The applicant must possess relevant fitness.
- For availing certain assistive devices, the applicant must possess a good life expectancy, which must be three years or more.
- The applicant must not be involved in any illegal activity or possess a criminal record

If a person cannot read and write, then he need not worry because he can take assistance from the representatives available in the Social Welfare Offices.
